{"id":45877,"date":"2022-07-22T12:00:07","date_gmt":"2022-07-22T11:00:07","guid":{"rendered":"https:\/\/onecontact.health\/template\/?page_id=45877"},"modified":"2024-02-20T12:04:05","modified_gmt":"2024-02-20T12:04:05","slug":"new-patient-registration-uwhc","status":"publish","type":"page","link":"https:\/\/pt.onecontact.health\/template\/new-patient-registration-uwhc\/","title":{"rendered":"New Patient Registration &#8211; UWHC"},"content":{"rendered":"\n<div class=\"wp-block-nhsblocks-panel1 is-style-panel-with-label nhsuk-card\"><div class=\"nhsuk-card__content\"><h2>New Patients Only<\/h2><div class=\"nhsuk-card__description\">\n<p><strong>You can be registered at the University of Warwick Health Centre if:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You\u2019re a current student at the University of Warwick.<\/li>\n\n\n\n<li>You\u2019re an Undergraduate living on Campus, within the catchment Area or in Kenilworth or Leamington.<\/li>\n\n\n\n<li>You\u2019re a Postgraduate living on Campus or within the catchment Area.<\/li>\n\n\n\n<li>You\u2019re a member of university staff living on Campus.<\/li>\n<\/ul>\n\n\n\n<p><strong>If you registered last year, you do not need to register again<\/strong><\/p>\n\n\n\n<p><strong>Health Centre Registration is part of the Enrolment Dashboard &#8211; PLEASE DO NOT REGISTER IF:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You are doing an On-line Course<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You want to stay with your home GP (if you are not registered you will not be eligible to be seen at Warwick Health Centre unless it is a Medical Emergency)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You registered the previous year and haven\u2019t registered elsewhere<\/li>\n<\/ul>\n\n\n\n<p><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-emergency-red-color\"><strong>Due to the number of registrations received at the start of each academic year, we will start to register new patients from the 2<sup>nd<\/sup>&nbsp;week of September.<\/strong><\/mark><\/p>\n\n\n\n<p><strong>Students or members of staff with children<\/strong>\u00a0need to register at the Health Centre in person. Please provide identification (Passport) and full immunisation history in English.<\/p>\n\n\n\n<p><strong>Dependants<\/strong>\u00a0&#8211; Only Spouses or Children of students\/staff can register at the Health Centre, parents and partners will need to register at another surgery.<\/p>\n\n\n\n<p><strong>You will be informed of the practice you are registered with on your first contact with the Health Centre.&nbsp;Please do not ring or visit to ask for this information<\/strong><\/p>\n\n\n<p>This form is currently closed for submissions.
